Pet Wellness Clinic

The Pet Wellness Clinic is hosting a Health Day for pet owners and breeders on Saturday, November 4th from 9am – 3pm. The registration form can be found here.  Please fill out your information at the top and select the tests you would like for your dog(s) below.  The registration form must be completed and emailed or faxed back to The Pet Wellness Clinic by Friday, October 27th.

We will send you an email acknowledging receipt of your registration–if you do not receive an email back from us, please call to confirm we got it.

Our events fill up fast, typically in the first week!   Be sure to get your registration forms returned in a timely manner.

To optimize your check in process on the day of the event, please download, print and fill in (or you can type the form online, look up our clinic, and the print the typed form) the OFA forms for the tests desired. (**FORMS FOR THE BASIC CARDIAC EXAM ARE NOW THE ONES OFFERED ON THE OFA WEBSITE, NO LONGER FILLED OUT IN PERSON**)   Bring these with you, including the separate payment to OFA if needed (fees are listed at the bottom of the forms).  Forms for eye tests (CAER’s) will need to be filled out in person at the clinic, not online.

Please have your forms (except eye tests) filled out before arriving.  This saves time, helps eliminate errors, and keeps our schedule on track. Printed forms are preferred, much easier for us to read and enter information into the computer.  Please sign the forms and fill out the credit card information at the bottom.  You can pay OFA with a check, but there is a mail delay when sending checks, credit card payments will be processed faster.

We had an unfortunate situation where a breeder forged a doctor’s signature and falsified results on an application form.  Therefore, a new policy is in effect:  All forms (except those for eye exams) will now be mailed to OFA by the clinic; no signed forms will be given out.   We can supply the owners with a form showing results for their records if desired. 

Dr. Mike Jewell will perform OFA radiographs and patella luxation checks; Dr. Brenda McClelland will perform various blood tests (including OFA thyroid), and microchips.  Dr. Derek Hanes (cardiologist) will be doing cardiac auscultations and Dr. Trevor Arnold (ophthalmologist) will be doing CAER (formerly CERF) exams. 

Please see the registration form for the tests available.  If there is a test you need that is not listed, contact us and we can check if it’s possible to perform.

All health day participants will be informed of their appointment time on Thursday, November 2nd or Friday, November 3rd!

COVID PRECAUTIONS: Currently, masks are not required for clients in the building.  The staff may be wearing masks. Hand sanitizer is available in the lobby. The specialists may or may not allow owners in exam rooms with them, they will determine that as the day approaches.  The number of people in the building may still be limited so please bring a vehicle with air conditioning/heat and there will be areas outside in which to sit as well.
